IME, PC. would like to make it convenient for you to
order services from us. Please complete either the Mail,
Fax, or Online Form for the services that most interest
you.

Upon receipt of the completed service form, a representative
from Independent Medical Evaluations, PC. will contact
you to verify your order, answer any questions, and
discuss any specific issues that you may have. If
you have not been contacted within 48 hours of submitting
your service order form, please contact us to ensure
that we received your transmission.
